Use with any EHR
It’s a reference you simply paste into your note. Any electronic medical record (EHR) that allows you a place to put free text can accommodate the clinical decision tools at soapnote.
The anatomy of a soapnote page
The best way to find what you need is usually the search tool at the upper right hand corner of the navigation bar. An indexing system of tags and categories can also help you sort through what’s available. Finally, all soaps are archived by date and can be seen under “recent posts” (link).
The lefthand side
Almost every page on the site will have a Lefthand side that has the following elements.
1. Log in/out
Registering and Logging On will enable many of the advanced functions which can make the site a more valuable clinical resource.
2. Favorite 10
These are the 10 most “favorited” Soapnotes.
3. Most favorited posts page
This page gives the tally of all the soapnotes in order of the number the times they were favorited by other users (most-favorited at the top)
4. My favorite soapnotes
If you are logged in, this is displayed. It is a list of the Soapnotes you have selected as “Favorites”. To select a Favorite, simply click on the heart icon when it is displayed.
5. My Soapnote Page
Clicking on this link will take you to your own author page. This has a categorical list of your favorite posts followed by a categorical list of your own posts.
The middle of the page
The middle/center part of each web page may be very different. The home page has the following elements.
1. Template Summary
This has background information regarding a particular template. This information is *NOT* copied when you click on the copy button. It is only intended to help you decide if this template meets your needs. It is composed of three (3) parts: Highlights, References, and Links. For those authoring and submitting templates, Template Summary information is not required. You may add it right away, later, or never. Highlights, references, and links found in the Template Summary can refer to the evidence that bolsters a particular template or calculator.
2. Template
This is the decision rule, or calculator you were (hopefully) looking for.
3. Copying and pasting output
Once a user has completed a form and submitted it, text is sent to the output box and is ready for use. The output box text may then be copied to the Electronic Health Record. Copying the output and pasting it in the EHR is the most straightforward way to use the text. Clicking the “copy” button takes the template text and puts it on the clipboard.
4. Steal this soapnote
You may embed a soapnote into your website by using this code. A registered user can click on the link to copy the code. Then just paste it into your site’s html code.
5. Rate this soapnote
A “Thumbs Up or Thumbs Down” is visible at the bottom. This may also be seen on may other pages in the site. Clicking on these will enter a rating for that page. This will affect the “Top 10″ listings over at the righthand side.
6. Bookmark this soapnote
This allows you to save frequently used soapnotes for future use. It also saves it to your my soapnote page.
7. Discuss this soapnote
Here users can leave feedback and suggestions regarding a particular template. A good place to notify if errors are found, as well.
The righthand side
Almost every page on the site will have a Righthand side that has the following elements.
1. Top 10
These are the 10 most highly rated (Thumbs Up) soapnotes.
2. Highest rated posts page
This page gives the tally of all the soapnotes in order of their ratings (best at the top)
3. Newest 10
These are the 5 most recently posted soapnotes.
4. Recent soapnotes page
This page has all soapnotes in chronological order, with most recent at the top.

index page
All soapnotes are indexed by categories, tags, and date. There is a page for each of these archives.
author pages
Clicking on a user’s name will take you to the page of soapnotes that they authored.
A variety of clinical decision tools
boilerplate
These are Soapnotes with text that need minimal modification but are used regularly. Paste text into your EHR and make edits as needed.
fillable templates (“***” & “—”)
These are more interactive Soapnotes which allow the user to “tab” through the text looking for elements that need replacing or editing. Clicking on the “fillable” button moves you to the next “***” or “—” in the text. Clicking on the “line item” button will highlight all of the text in a line after “+++”. Finally, clicking on the “clause item” button will highlight all of the text between two sets of “###”. Using these tools you can get to the place(s) that need editing quickly to build your note.
prompts
These Soapnotes provide you with choices as you build the note. Multiple choice, checkboxes, and fill-in-the-blank fields are used to populate the text of the note.
prompts with conditional input
In these interactive Soapnotes, the input provided determines the prompts downstream. For example, if “male” is selected, all of the questions relevant to females will be hidden.
calculators
Calculators on soapnote allow you to paste not just the calculation, but the decision-making process, as well. The site is optimized to apply point-based tools developed by multivariate analysis.
