Here are two calculators which I’m using at an IHS clinic with a heavy prevalence of diabetes.

http://www.soapnote.org/endocrine-metabolic/oral-diabetes-medications/ is one that I’m using to figure out what my patients can take as I try to drive down the A1cs on the way to insulin.
