Bronchitis assessment/plan

***Bronchitis/Acute bronchitis/Chronic bronchitis/Exacerbation of COPD/Productive cough/Cough***
Antibiotics:
None indicated
Amoxicillin 500 mg tid x 10 days
Bactrim DS 1 bid x 10 days
Biaxin 250 mg bid x 10 days
Biaxin 500 mg bid for 10 days
Erythromycin ES 400 mg four times a day for 10 days
Levaquin 500 mg q day x 10 days
Vantin 100 mg/5cc ***dose*** bid x 10 days
Vantin 100 mg bid x 10 days
Vantin 200 mg bid for 10 days
Zithromax 100 mg/5cc ***dose*** now, then ***dose*** q d x4 days
Zithromax 200 mg/5cc ***dose*** now, then ***dose*** q d x4 days
Zithromax 500 mg now followed by 250 q d x4 days
Additional symptomaic treatments:
Albuterol inhaler 1-2 puffs q 4-6 hours prn
Maxair inhaler 1-2 puffs q 4-6 hours prn
Phenergan with codeine 1-2 tsp q 4-6 hours prn cough, 4 ounces
Robitussin AC 4 ounces 1-2 tsp q 4-6 hours prn cough
Robitussin AC 8 ounces 1-2 tsp 1 4-6 hours prn cough
Tessalon Perles 100 mg 1 tid prn cough
symptomatic measures were discussed
recheck if not better with treatment or sooner if worse
reviewed with patient potential side effects/interactions of medications.
increase fluids
over-the-counter antitussives prn.
follow up prn
follow up promptly if worsening
patient advised to quit smoking
