hypertension HPI Sample
S: F/u HTN, pt has concerns about nifedipine medication since his dose was recently increased from 30mg to 60mg daily on 04 Oct. Patient says he cannot remember to take two of the 30mg pills and would like the labels on the pill to properly reflect what he needs to take. Pt has no further concerns about HTN. - home readings: 10-8 130/54 10-9 122/62 10-10 128/66 10-11 140/60 10-12 150/50 - Denies CP, SOB, ankle swelling, palpitations, dizziness, PND, orthopnea, vision changes or neurological deficits - No FamHx early cardiac disease, personal hx cardiac disease Current BP Meds: adherent, denies s/e at this time. -Nifedipine 60mg, once daily. Patient has Nifedipine 30mg tablets and instructed to take two pills, once daily. -Telmisartan 40mg, once daily.
