86 min spent reviewing hospitalization H&P, progress notes, imaging, labs, D/C summary. pt w/complicated medical hx w/multiple comorbidities. Reviewed PHMx, past surgical and social history, medication allergies, and confirmed D/C medications and plan of care with nursing. also time spent ROS, examining the patient, developing a plan of care and discussing w/patient, coordinating care with RNs, skilled therapeutics w/med reconciliation, and writing the new orders for patient.
