_-month old _ here for well child check. No parental concerns at this time.

– Diet: No concerns.
– Voiding/stooling: No concerns.
– Sleeping: Has regular bedtime routine, and sleeps through the night without feeding.
– Behavior: No concerns.
– Activity: Screen/TV time is limited to < 2 hrs/day.

PM/SH:
Normal pregnancy and delivery. No surgeries, hospitalizations, or serious illnesses to date.

DEVELOPMENT:
– Gross motor: Pulls self to a stand, cruises. Starting to walk.
– Fine motor: Uses pincer grasp, feeds self, bangs toys together, drinks from a cup.
– Cognitive: Follows simple directions, hands adults books to read.
– Social/Emotional: Laughs, likes to play games (e.g. “peek-a-boo”), + stranger anxiety, looks at parent when name is called, waves bye-bye, tries to copy adults and older children.
– Communication: Knows 1-2 words, babbles, imitates sounds, uses gestures.

SOCIAL:
– No smokers in the home.
– No concerns regarding postpartum depression.
– No major social stressors at home.
– No safety concerns in the home.
– Daytime child care is with _
– No TB or lead risk factors.

IMMUNIZATIONS:
– Up to date.
