*   Inquire about sleep patterns: Hours of continuous sleep and daytime naps.
*   How many hours does the pt spend watching television, listening to the radio, playing video games, or playing on the computer?
*   Has the pt had any accidents?
*   Does the pt attend day care or school?
*   If the pt is in school, how is he or she performing?
*   Is the child participating in extracurricular activities or organized sports?
*   Are there any behavioral issues or misconduct?
*   If female, has the pt started her menses?
PMH: Review Prenatal History and Birth History
*   Hospitalizations: Any emergency room visits or hospitalizations?
*   Immunizations: Are any immunizations missing?
PSH: Any surgeries or special procedures since last visit?
Allergies: Any known drug allergies?
Medications: Using prescribed or over-the-counter medications?
FMH and SH: 
*   Check vital signs, 
Check height, weight, and head circumference charts for age-appropriate growth.
