* Address parental concerns. * Did the pt have any reaction to immunizations given at last visit? * Has the pt had any recent illness? * Ask about diet and dietary habits: Timing of feeds or meals. * What are the pt's elimination or voiding patterns? * Inquire about sleep patterns: Hours of continuous sleep and daytime naps. * How many hours does the pt spend watching television, listening to the radio, playing video games, or playing on the computer? * Has the pt had any accidents? * Does the pt attend day care or school? * If the pt is in school, how is he or she performing? * Is the child participating in extracurricular activities or organized sports? * Are there any behavioral issues or misconduct? * If female, has the pt started her menses? PMH: Review Prenatal History and Birth History * Hospitalizations: Any emergency room visits or hospitalizations? * Immunizations: Are any immunizations missing? PSH: Any surgeries or special procedures since last visit? Allergies: Any known drug allergies? Medications: Using prescribed or over-the-counter medications?
