[text name="variable_3" default="Unit #"] arrived to find the patient [select name="variable_4" value="alert and oriented|Alert to self|Altered at baseline"]. The patient was [select name="variable_2" value="assisted|transferred via draw sheet"] to the stretcher, placed in a position of comfort, and secured with three straps. The patient was then placed in the unit where they were assessed. Baseline vitals were found as listed in the ePCR software. The patient was monitored en route with minimal trending. On arrival at [text name="variable_2" default="sending"] the patient was placed in their bed via sheet pull and care was transferred to [select name="variable_3" value="Nursing staff|family"] without incident. [text name="variable_3" default="Unit #"] cleared the scene and returned to service without incident.
