Oral Surgery consult. The dental team verified that external and internal indicators were appropriately exposed to the critical parameters of sterilization before use in patient care. Medical History Rev. Xr rev. Extraoral and Intraoral exam Performed. Floor of mouth, tongue and oral soft tissues without pathosis. Dx: [text name="variable_1" default="sample text"] Discussed surgical indications, procedures, risks, benefits, possible complications to include but not limited to pain, swelling, infection, bleeding, sinus involvement, alveolitis, adjacent tooth damage, bone fx and nerve injury resulting in numbness of lower lip, teeth, gums, tongue, or loss of taste in the tongue; temporary or permanent. Alternative tx discussed to include no tx. Discussed IV sedation/GA as anesthesia adjunct to proposed procedure; pt desires sedation. Pre-anesthesia assessment complete. See Sedation Monitoring Form.
