ENDOCRINOLOGY/DIABETIC CONSULT OUTPT
Diabetes

Please refer for the management of complicated diabetes. PACT teams should 
manage most diabetics, including insulin initiation/titration and glucometer 
issue/instruction. 

Please do not refer noncompliant patients, patients refusing 
insulin, patients on submaximal doses of oral medications, and 
straightforward nonformulary requests. Uncomplicated nonformulary requests 
should be submitted directly to pharmacy with appropriate justification.

Endocrine

For osteoporosis: please obtain DXA scan (available under general radiology), 
vitamin D level, and creatinine

For hypogonadism: please draw total testosterone, SHBG, LH/FSH

For thyroid disorders: please draw TSH and free t4

For thyroid nodules: please order thyroid (neck) ultrasound
