Polymyalgia Rheumatica Diagnostic Criteria - Healey
Pain:  To meet criteria, 2 of the following 3 areas below must have pain

[select name="steroid" value="no=0|YES=1"] <-- Rapid response to prednisone (20 mg/day or less)
[select name="esr" value="no=0|YES=1"] <-- Erythrocyte sedimentation rate (ESR) > 40 mm/hour
[select name="excluded" value="no=0|YES=1"] <-- Absence of other diseases capable of causing the musculoskeletal symptoms
