AMA

In spite of multiple attempts by myself and staff to convince the patient to stay for evaluation and treatment, we have unfortunately been unsuccessful. However, the patient has the capacity to give, receive, and withhold information. The patient verbalizes understanding of their condition and symptoms and that this represents a significant threat. The patient has verbalized to me that they understand the plan of diagnosis and treatment, and unfortunately will not agree and understand that it may cause worsening of their condition or death. The patient understands that they can come back at any time for further care without prejudice and we will be happy to provide treatment again.
