PHYSICAL EXAM

GENERAL: Alert and oriented x 3. No acute distress. Well-nourished.
EYES: PERRL. Anicteric.
HENT: Moist mucous membranes. No scleral icterus. No cervical lymphadenopathy.
CHEST:  Symmetric chest expansion.  Lungs clear to auscultation bilaterally. No accessory muscle use.
CARDIOVASCULAR: Regular rate and rhythm.  +3/4 harsh early diastolic murmur loudest over the left 4th ICS at mid-clavicular line and radiating to the axila. No JVD.
ABDOMEN: Soft, non-tender and non-distended. No palpable masses.
PELVIC: Normal circumcised penis with scant dried blood around the urethral meatus.
EXTREMITIES: Mild 1+ pitting edema of the LLE.  No RLE pitting edema.  No warmth or erythema.  Non-tender.
SKIN: No rashes or lesions. Warm and dry.
NEUROLOGIC: No focal neurological deficits. CN II-XII grossly intact, but not individually tested.
PSYCHIATRIC: Mood is euthymic and affect is appropriate.
