It is always free to make your own SOAPnotes. If you need help along the way, you can check out the [link url="https://www.soapnote.org/user-guide/beginning/" memo="User Guide"] or leave a message at the [link url="https://www.soapnote.org/help/" memo="Help Page"]. But, if you have an idea, some text, or a rough draft that you would like made into a SOAPnote, consider having us do the work for you. Whether it's a note template, a checklist, a customized calculator, or a special form, we can help!
For private SOAPnotes, the cost is $10 for the first 20 lines of SOAPnote Markup and then $10 for each additional 20 lines. For public SOAPnotes, the cost is $5 for the first 20 lines of SOAPnote Markup and then $5 for each additional 20 lines. It may help you gauge the number of lines to know this form you are reading is 20 lines long (a line ends with a carriage return). Please note: You must be a registered user of the site (it is needed so we can save the note in your account). Also, the note, template, checklist, or calculator must be possible within the existing limitations of the soapnote.org website.
After you fill out this form, please copy the text or email it using the button below. We don't receive the form unless you take the step of sending it by email! If you're not quite ready and just have questions, you can send them to [link url="mailto:orders@soapnote.org" memo="orders@soapnote.org"], as well.
