Based on today’s evaluation, patient meets diagnostic criteria for [comment memo="DIAGNOSIS" memo_size="large"] [select name="variable_criteria" value="Major Depressive Disorder due to symptoms of depressed mood most of the day, nearly every day for more than 2 weeks, along with little pleasure in doing things, hopelessness, helplessness, lack of energy, poor concentration, changes in appetite, psychomotor retardation and poor sleep which greatly impact patient’s everyday life|Generalized Anxiety Disorder due to symptoms of anxiety about multiple aspects  patient’s life and difficulty controlling worry, nerly everyday, for a minimum of 6 months, along with anxiety;
