AMA/Discharge Prior to Completion of Workup

The benefits of continued admission have also been explained, including the availability and proximity of nurses, physicians, monitoring, diagnostic testing, treatment and [text default="" size="60"]. The patient was able to understand and state the risks and benefits of hospital admission. This was witnessed by [text default="" size="60"] and me.

They had the opportunity to ask questions about their medical condition. The patient was treated to the extent that they would allow and knows that they may return for care at any time. Follow-up has been emphasized with plan to include [text default="" size="60"]. It has been stressed to the patient that they may return at any time for additional diagnostics or concerns. At the time of leaving AMA pt is in [checkbox name="condition" value="stable|guarded|critical"] condition.
