Plan – Anaphylactic Shock

 IMMEDIATE CARE
• Epinephrine 
0.1 mg (1 mL of a 1-mg ampule of epinephrine diluted in 10 mL saline IV; may be repeated q 5-10 min

• methylprednisolone 
250 mg IV

• Volume replacement infusions
5% human albumin or plasma protein solution). 
Colloid volume replacement solutions are contraindicated due to their potential allergenic effects

• diphenhydramine 
25-50 mg IV

• Clear airways for ventilation, in the event of laryngeal edema: intubation or tracheotomy

• Occasionally required in case of severe, persistent bronchial spasms: theophylline - ethylene diamine (aminophylline) 6 mg/kg as slow (10-20 min) IV drip
