General - no fever, night sweats, unexplained weight loss
Skin - no skin lesions, changing moles, wounds, hair changes
Hematologic/Lymphatic - no bleeding, bruising, swollen lymph nodes
Musculoskeletal - no joint pain, swelling, or stiffness, cramps, weakness
Endocrine - no skin or hair changes, intolerance to heat or cold, fatigue, tremor, polydipsia, polyphagia, polyuria
Allergic/Immunologic - no seasonal allergies, known allergies or sensitivities, history of anaphylaxis
HEENT - no headache, vision changes, hearing loss, vertigo, nose bleeds, sinusitis, dysphagia, hoarseness
Breast - no lumps, skin changes, discharge
Respiratory - no cough, shortness of breath, wheezing, tobacco use
Cardiovascular - no angina, edema, orthopnea, exertional dyspnea, paroxysmal nocturnal dyspnea, syncope
Gastrointestinal - no abdominal pain, nausea, abnormal stools, diarrhea, blood in stools, melena, history of abdominal surgery
Genitourinary - no dysuria, incontinence, increased urinary frequency, nocturia
Neurological - no seizure, weakness, syncope, numbness, radiating pain
Psychiatric - no increased or decreased sleep, mood changes, anxiousness, panic attacks, decreased energy, forgetfulness, hallucinations, suicidal or homocidal ideation[/textarea]
